DWI – OASAS Screening and Assessment
Oasas approved provider for the DWI – OASAS Screening and Assessment requirements. For Individuals charged with or convicted of an impaired driving offense who require clinical screening and assessment services for DMV violation or for legal substance issues.
We provide evaluations for people mandated by a judge/court or suggested by a lawyer as a result of a DWI or other impaired driver offense.
We are not the treatment center so we give a fair and honest assessment.

What To Expect?
Assessment is done by a certified DWI – OASAS Screening and Assessment Provider in accordance with changes in the State’s Vehicle and Traffic law (VTL),
Assessment consists of a Clinical face-to-face, in-person interview and collateral interviews with significant others. A psychosocial history is obtained to include presenting problem, including a discussion of pertinent and detailed information describing contributing factors, incidents and issues that precipitated assessment.

What You Need.
- A copy of their police report
- A copy of any court orders, citations or a breathalyzer report
- Undergo a urine drug screening which is performed by a qualified laboratory licensed pursuant to Public Health Law § 575.

Please print out 3 copies. (4 if you have a layer) On each form please fill line 6 with the name and phone number of someone we can contact to discuss you current situation. This is a requirement of IDS that we contact 3 collateral people. We can not speak to anyone without this form signed.
